How to Request a Mobile Device

Verizon Users only

A user who has never been issued a university cell phone must complete the following 2 forms and have signed.

1. User and department must complete the Mobile Request form with their Budget person's signature for authorization. Must include their U900 cell phone account.

  • This is the request for the user
    • Type phone
    • Free phone or budget authority phone
    • Basic necessary accessories (case and adapter)
  • Monthly billing form

2. User and department must complete the MCD (Mobile Communication Device) form. No allowances are given.

  • This requires 3 signatures for university approval
    • User, Supervisor and the Dept V.C.

Please Note:

  • All departments do not have Mobile Device accounts, the existing were grand-fathered in. 
  • Telecom can verify, if your department does not have one on file.
  • If not, you will need to speak with your dept chair for a resolution.
